Leave of Absence application
​
You may access the Leave of Absence Application Procedure on page 15 of the COE Online Processing of Documents.
-
Apply Leave of Absence (LOA) online using your crs account.
-
Email your department for adviser’s endorsement.
-
Answer this form.
-
Student will receive a confirmation email of the request.
-
Admin will verify/check if you’re eligible to file for LOA.
-
Once approved, Admin downloads the LOA form from CRS and affixes the appropriate signatures;
-
The admin then sends the signed LOA form to the student with the payment details and OUR payment slip.
-
Student and the parent/guardian (if necessary) sign the LOA form.
-
Student pays over-the-counter (OTC) at any open Landbank branch or avail of fund transfer feature of his/her bank to Landbank (Note: If paying via fund transfer, please use InstaPay as PESONet is not Real-time) ;

Student emails the signed LOA form, photo/scanned copy of the proof of payment, and the filled out Payment Slip to admission.our@upd.edu.ph with the subject: Bankpayment_LOA_name for LOA application or Bankpayment_LateLOA_name

For Late Application of LOA,

Make an appeal letter for late filing of LOA addressed to Dr. Maria Vanessa Lusung-Oyzon, University Registrar, UP Diliman.

Answer this form and upload the soft copy of the appeal letter

Student will receive a confirmation email of the request.

The Admin will send you back your letter of appeal duly endorsed by the College Secretary.
-
Send your appeal to admission.our@upd.edu.ph for the university registrar’s approval.
-
OUR will notify you on the status of your appeal.
-
Once approved, apply Leave of Absence (LOA) online using your CRS account.
-
Email your department for adviser’s endorsement.
-
On the same email thread of your confirmation email from enggadmin, email enggadmin regarding your application for LOA.
-
Admin will verify/check if you’re eligible to file for LOA.

The Admin will send you back your appeal letter duly endorsed by the College Secretary and other proof of enlistment/enrollment.

Student emails the OUR (our.diliman@up.edu.ph) about his/her request for late registration/CoM and requests for approval preferably using his/her UP mail account to lessen the verification process;

Once approved, the OUR sends a soft copy of the fillable Payment Slip to the student

APPLICATION OF UNIVERSITY CLEARANCE

Student applies for a University Clearance in his/her CRS account (crs.upd.edu.ph);

Student emails the rcs_our.updiliman@up.edu.ph to request for a Payment Slip with the subject: Clearance -Request for Payment Slip_studentno (Note: Kindly indicate in the email if the student is a graduate of a certificate program to properly assess the student);*Note: Student who were eligible of RA 10931 or Free Tuition during their last enrollment (effective 2nd Semester 2019-2020) will no longer pay the P330 graduation and clearance fees.

Registration and Clearance Section (OUR-RCS) sends a soft copy of the fillable Paymeny Slip to the student;
-
Student pays via fund transfer feature of his/her bank to Landbank or pays over-thecounter (OTC) at any open Landbank branch (Note: If paying via fund transfer, please use PESONet);

Student waits for the confirmation email that the fund has been transferred (proof of payment);

Student sends back the scanned/photo of the proof of payment and the filled out Payment Slip to rcs_our.updiliman@up.edu.ph;

If everything is in order, OUR-RCS sets the student’s application as “paid”;

Student monitors the status of his/her clearance application in his/her CRS account;

Once cleared, OUR-RCS forwards the student’s clearance to the Transcript Section (if applying for a TOR, COG, EMI, etc.). Please note that the student needs a separate application to process this. (Click here to know more)
